Your Care Coordinator works as an extension of your doctor’s team. Your physician remains the primary decision-maker, while the Care Coordinator provides additional insight to help ensure you, your doctor, and all your healthcare providers have the information needed for effective care.
Your Care Coordinator has a broad view of your overall health along with the important details. They work closely with everyone involved in your care including your family when appropriate to understand your needs and make sure you receive the right care, at the right time, from the right provider.
The goal of this program is to keep you well and address potential concerns before they become more serious. It also helps reduce unnecessary or duplicate services.
